FORMS:

All students must complete a Student Athletic Participation Packet. This includes a student information sheet (salmon colored), WIAA School Eligibility Checklist (Lavender colored) and a WIAA preparticipation history and Physical Examination.

FEES:

All students must pay a participation fee ($65.00 Sports, $15.00 Activity)

To be eligible to participate in Sports and/or Activities at PHS:

• Student is currently enrolled half-time or more
• Student had a 2.0 GPA or above on the last transcripted grading period and failed no more than one class.
• Student must reside with parent(s) or legal guardians in this high school attendance area.
• Student must be enrolled in a school program and in regular attendance the first 15 days of the current term.
• Student must pass a physical exam during the last 24 months. A copy must be on file in the Athletic Office.
